29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) 149 Plainfield St, Chicopee, MA: On May 19, 2022 and at times prior, the employer had not performed triennial performance evaluations to ensure powered industrial truck operators were safe and proficient. PIT operators received initial training but operator performance was not evaluated every three years.
