29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a. On or about May 6, 2022, the employer failed to use LOTO devices during group LOTO. This LOTO failure resulted in an maintenance employee being hospitalized due to a fractured leg while performing paddle replacement on the paddle conveyor in the paddle conveyor area. Inca Presswood Pallets, LTD, was previously cited for a violation of this occupational safety and health standard 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1313711, citation number 1, item number 3/b and was affirmed as a final order on September 5, 2018, with respect to a workplace located at 3005 Progress Street Dover, OH 44622.
