29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Cherry Hill Rehab and Care Center LLC dba Dwellside Care and Rehab, as successor to Avista Healthcare, LLC 3025 Chapel Avenue, Cherry Hill, NJ 08002: On or about 1/2/2022, the employer did not record a work-related illness of a Licensed Practical Nurse who contracted COVID-19 on the OSHA 300 log or its equivalent. b) Cherry Hill Rehab and Care Center LLC dba Dwellside Care and Rehab, as successor to Avista Healthcare, LLC 3025 Chapel Avenue, Cherry Hill, NJ 08002: On or about 1/3/2022, the employer did not record a work-related illness of a Certified Nursing Assistant, a temporary employee, who contracted COVID-19 on the OSHA 300 log or its equivalent. c) Cherry Hill Rehab and Care Center LLC dba Dwellside Care and Rehab, as successor to Avista Healthcare, LLC 3025 Chapel Avenue, Cherry Hill, NJ 08002: On or about 1/10/2022, the employer did not record a work-related illness of a Certified Nursing Assistant who contracted COVID-19 on the OSHA 300 log or its equivalent. Cherry Hill Rehab and Care Center LLC dba Dwellside Care and Rehab, as successor to Avista Healthcare dba MIMA Healthcare was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.4(a), which was contained in OSHA inspection number 1472865, citation number 2, item number 1 and was affirmed as a final order on 1/7/22, with respect to a workplace located at 3025 Chapel Avenue, Cherry Hill, NJ 08002.
