29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: On or about April 14, 2022, the employer did not provide a fit test to all employees required to wear respirators on an annual basis. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents. Omni Manor, Inc. dba Windsor House at Omni West Assisted Living was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1509975, citation number 1, item number 1 and was affirmed as a final order on June 11, 2021, with respect to a workplace located at 3259 Vestal Road, Youngstown, OH 44505. Abatement was previously required and submitted for this violation.
