The certification did not contain each employee's name and dates of training. At 1776 Cecos Lane, Arnold, MO, 63010, On April 11, 2022 and at times prior thereto, employees were exposed to hazards associated with the unexpected energization of equipment due to the employer not certifying that training had been accomplished and kept up to date. Training records with names and dates were not available.
