Washington Senior Living, LLC located at 1201 Newcastle Road in Washington, IL. On or about April 8, 2022, the employer failed to ensure the COVID-19 log contained, one form of contact information, occupation, the date of the employee's last day at the workplace and the date of the positive test for, or diagnosis of COVID-19.
