Ellwood City Forge Company, Machine shop lay down area/Mezzanine area, on or about April 4th, 2022 and times prior thereto: A safety label that was attached to the remote pendant labeled "Mezzanine" was defaced/not legible due to removal of a portion of the warning label. The remote pendant was associated with the Zenar brand 20 Ton crane (Serial#8648).
