29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about March 29, 2022, and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to die rams, drive shafts, universals, rockers, and pusher assemblies on: - Reed thread rollers, model B-112, (K27-3) (R1) and (K-27-4) (R3). - Waterbury thread rollers, models Hand #20 (K11-2) (A2); Hand #30 (K12-1) (A9), (K12-2) (A8), (K12-4) (A7); Hand #40 (K15-1) and (K15-3); and Hand #60 (K30-2); and, - Tesker thread rollers, T1 (K27-5), T2 (K27-6), T3 (K27-7), and 45-2. b) On or about April 12, 2022 and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to, kick areas, infeed rollers, flatteners, straighteners, wire feeders, and progressive dies on Headers #50 (A97-2), #44 (A97-1), #41 (A96-1), and Jern Yao. c) On or about April 12, 2022, and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the in-going nip points of rotating abrasive wheels likely to result in serious injury on Centerless grinders, Cincinnati #2 G07-16.
