29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees grease the forge welder daily and are exposed to pneumatic and electrical hazards. b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees change the dies on the C Frame-Press and are exposed to electrical and hydraulic hazards. c. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that maintenance employees lock out equipment with electrical, pneumatic and hydraulic hazards. Columbiana Boiler Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(2)(ii), which was contained in OSHA inspection number 1472654, citation number 01, item number 001A and was affirmed as a final order on February 11, 2020, with respect to a workplace located at 200 West Railroad Street, Columbiana, OH 44408.
