29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: Elevator 905 ( State ID 1-P-9676 ) Hazardous mechanical energy was not adequately controlled when employees were servicing the elevator from the car top: Instance A: On January 28, 2022, an employee working from the car top was severely injured when the car moved up and the employee was caught between the elevator car cross-head and the top of the hoistway. Instance B: On or about January 31 - February 4, 2022, employees working on the car top did not adequately control potentially hazardous mechanical energy while performing repair work on the elevator. Instance C: Employees posting 'low clearance' warnings on the cap top on or about March 4, 2022 were on top of the elevator car whose potentially hazardous mechanical energy had not been adequately controlled.
