29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. (a) Dairy Resources of the West, LLC DBA Dairy Resources, 17868 County Road 15, Platteville, CO 80651: On or before March 9, 2022 the employer did not report the in-patient hospitalization of an employee that broke his leg while performing maintenance of a rotary parlor. The employee was admitted to the hospital on Marc 7, 2022, and the employer did not report the hospitalization to OSHA until March 9, 2022.
