29 CFR 1910.132(d)(2):The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: Building 24760: On and/or before March 3, 2022 the Job Safety Analysis (JSA) is a procedure built in place to recognize any potential hazard or accidents, and it specifies steps in order to prevent and/or eliminate hazard(s). The JSA, used specifically for the custodians and the maintenance department, were not verified by the employer, dated correctly, and/or specified workplace location other than building 24760. Employee(s) were exposed to electrical, fire, fall, and/or use of tool hazard(s).
