29 CFR 1904.32(b)(3): A company executive did not certify that he or she had examined the OSHA 300 Log and that he or she reasonably believed, based on his or her knowledge of the process by which the information was recorded, that the annual summary was correct and complete. On or about March 3, 2022, the OSHA 300A logs for 2020 and 2021, Total Number of Cases section did not equal the Total Number of Injuries and Illnesses section after a company executive had certified that they were correct and complete.
