29 CFR 1910.119(d)(3)(i)(B): Process Safety Information pertaining to the piping and instrumentation diagrams (P&ID) was not compiled, complete and accurate. a) This violation was most recently observed on or about February 21, 2022, when the employer did not ensure Process Safety Information (PSI) such as Piping and Instrumentation Diagram (P&ID) was complete and accurate including, but not limited to, the flange connections for the 6-inch check valve located on the inlet pipe of the ejector ##215-2801. b) This violation was most recently observed on or about March 29, 2022, when the employer did not ensure Process Safety Information (PSI) such as Piping and Instrumentation Diagram (P&ID) was complete and accurate including, but not limited to, the permanent pressure gauge on the 6-inch flare header pipe.
