Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a-222 E Crescent Ave, Mahwah NJ 07430: (6 on 12 pitched roof) Employees engaged in sheathing activities activities on a 6 on 12 pitched roof was exposed to fall hazards of approximately 20ft to the asphalt below in which a means of fall protection was not utilized (Photo #8075).
