29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about 1/21/2022 at 12195 46th St N, Clearwater, FL, 33762, an employee suffered a work-related amputation. The employer was aware of the amputation on 1/21/2022. The employer notified OSHA on 1/31/2022 at 3:51 p.m. of the amputation.
