29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) At the site located at Across the street from 4595 Old Louisville Road, on or about January 28, 2022, the employer exposed employees to struck-by hazards in that the employer did not ensure the use of head protection for employees working in an excavation where concrete storm drain was being lowered into.
