29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) St. John's Riverside Hospital at 967 North Broadway Yonkers, NY 10701: On or about 01/10/2022 the employer upon being informed of an employee fatality on 01/09/2022 that may be a result of a COVID-19 related illness failed to report this fatality to OSHA within the 8-hour timeframe to determine work-relatedness. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE INSPECTION CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OF ABATEMENT FOR THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
