29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about January 10, 2022, when reporting a fatality after-hours to the OSHA 800 number, insufficient information was left causing a delay in OSHA's response. The employer did not report information including, but not limited to: (a) The location of the work-related incident; (b) The time of the work-related incident; (c) The name of the employee who suffered a fatality; (d) A contact number where someone could be reached, in that the number provided was the business phone and no one was available to answer at the time the fatality was reported.
