29 CFR 1904.41(a)(2): The establishment, which had 20 or more employees but fewer than 250 at any time during the previous calendar year and was classified as a high risk industry listed under Appendix A to Subpart E of this part, did not electronically submit information from the OSHA 300A, Summary of Work-Related Injuries and Illnesses, to OSHA or OSHA's designee once a year, no later than the date listed in paragraph (c) of this section: a) CCS Contractor Equipment & Supply, LLC dba Surebuilt, located at 840 S. 25th Avenue, in Bellwood, IL - On or about May 12, 2022, the employer did not electronically submit information from calendar year 2021's OSHA Form 300A or equivalent by March 2, 2022.
