29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about January 3, 2022, and at time prior thereto, the employer did not report to OSHA a work related hospitalization within the required 24 hours after the employer was aware of the hospitalization. The incident and hospitalization occurred on December 9, 2021 and was not reported until January 3, 2022.
