29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Mechanics were exposed to amputation hazards while working on a timing issue on an energized Gyoza Number 4, Daehan Machinery Co. Ltd. Model DHGZ-22, Serial Number 190218-10004 dumpling machine. The written procedure was not utilized and the machine was not shut down/locked out. Violation was observed on or about December 13, 2021. b) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A mechanic was exposed to amputation hazards while working on a maintenance issue on an energized Wonton machine, Daehan Machinery Co. Ltd, Model DHW-32. The written procedure was not utilized and the machine was not shut down/locked out. Violation was observed on or about December 13, 2021. c) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - An employee was exposed to amputation hazards while they were replacing a door interlock switch on an energized Ilapak Packaging Machinery Horizontal Packaging Machine, serial number 0642220241 as the company lockout / tagout program, revision 4 dated February 17, 2021, did not include any procedures for locking out or tagging out this machine . Violation was observed on or about February 17, 2022. d) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - An employee was exposed to amputation hazards while they were cleaning the floor on an energized Leepack RT-108 Rotary Packaging Machine as the company lockout / tagout program, revision 4 dated February 17, 2021, did not develop nor document procedures for locking out or tagging out this machine . Violation was observed on or about February 17, 2022. e) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - The Engineering Manager was exposed to amputation hazards while investigating a maintenance issue on the energized W.E. Plemons Machinery Services, Inc. Model #300800 Serial # 18-09-018009 Boxing Machine. The written procedure was not utilized and the machine was not locked out. Violation occured on or about February 17, 2022. f) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A mechanic was exposed to amputation hazards while working on clearing an obstruction the Daehyun Hi-Tec. Co. LTD, Model Number DH33ED, Serial Number 2K11712144. The written procedure was not utilized and the machine was not shut down/locked out. Violation was observed on or about February 18, 2022. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
