29 CFR 1904.10(a):The employer did not record all cases on the OSHA 300 Log when an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS. (a) Facility wide: On or about December 15, 2021, the employer did not record all cases on the OSHA 300 Log when an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS. The following three employees had not been recorded: 1. Establishment: Employee number 1 (Saw Room) suffered a Standard Threshold Shift 11.7 dB with a hearing level of 36 dB when averaged at 2000, 3000, and 4000 Hz range in the right ear and had not been recorded on the year 2021 OSHA 300 and 300A. 2. Establishment: Employee number 2 (Production) sustained a Standard Threshold Shift of 16.7 dB with a hearing level of 25 dB when averaged at 2000, 3000, and 4000 Hz range in the right ear and had not been recorded on the year 2021 OSHA 300 and 300A. 3. Establishment: Employee number 3 (Production) sustained a Standard Threshold Shift of 18.3 dB with a hearing level of 63 dB when averaged at 2000, 3000, and 4000 Hz range in the left ear and had not been recorded on the year 2021 OSHA 300 and 300A.
