The employer continues to fail ensure employees are protected from falls while they installed asphalt shingles to the roof top of a new build residential home. The most recent incident of such failure occurred on or about December 14, 2021, located at Lot 231, Granville, Ohio 43023: the employer failed to ensure that two employees were protected from falls when they conducted work on the roof of a new build residential home. The employees were exposed to a fall distance of 10 feet 2 inches to the ground level. Mr. �ngel P�rez, was previously cited for a repeat violation for this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1501959, citation number 1, item number 2 and was affirmed as final order on April 12, 2021, with respect to a workplace located at 174 Preserve Drive Lot 9 Pickerington, OH 43147 Mr. �ngel P�rez , was previously cited for a repeat violation for this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1460891, citation number 2, item number 3 and was affirmed as final order on April 09, 2020, with respect to a workplace locate at 6922 Duke Drive, Canal Winchester, OH, 43110. Mr. �ngel P�rez , was previously cited for a serious violation for this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335305, citation number 1, item number 3 and was affirmed as final order on October 24, 2018, with respect to a workplace locate at 6045 Kenzie Lane (Lot #44), Dublin, OH, 43017.
