29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: Location - Worksite at 1245 Auburn Street, Whitman: On or about December 16, 2021, the employer did not report the following fatality to OSHA that occurred when one (1) employee fell approximately 12 feet from the basket of an aerial lift to the ground and passed away eight (8) days later from injuries sustained in the fall.
