The employer does not protect each employee from struck-by hazards such as possible head injuries from falling and or flying debris from above. This most recently occurred on December 2, 2021, at a work-site located next to 4694 Wyoga Lake Rd, Stow, Ohio 44224; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 9.4 feet. ILS Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a) Protective Helmets), which was contained in OSHA inspection number 1356354, citation number 01, item number 01 and was affirmed as a final order on 1/7/2019, with respect to a workplace located at 6595 Brecksville Rd., Independence, Ohio. To abate this hazard in the future, the employer must ensure employees are provided with head protection as required by this standard. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps they are taking to ensure compliance.
