29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Entrance to Mass Pike Toll Plaza., Chicopee, MA: On or about November 29, 2021, an employee was observed working in an excavation, which was greater than 5 feet deep, without any protective system in place. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
