29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. The employer failed to report an inpatient hospitalization that occurred on November 16, 2021 at about 10:30 a.m. within 24 hours. OSHA was notified on November 19, 2021 at 12:00 p.m.
