29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Warehouse: On or about 10/22/2021, the employer did not report to OSHA within 24 hours the hospitalization of an employee resulting from a work related injury. The injury occurred on 10/22/2021 and was reported to the employer on the same date. The employer did not report the injury until 10/25/2021.
