29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about October 21, 2021, the employer did not report within 24-hours a work-related amputation when an employee experienced a multiple fingertip amputation when they made contact with the moving parts from a Hobart Chopper while clearing jammed pieces of meat.
