29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a)Water System Drilling, Inc. at Box Elder City Well #10 site, Box Elder, SD 57719: On or about and at times prior to October 16, 2021, the employer did not ensure that a seven and a half finger amputation that occurred on a water well drilling rig and resulted in an in-patient hospitalization was reported within 24 hours.
