Located at 8500 Green Meadows Drive N, Lewis Center, Ohio: a. On the 2021 OSHA 300 log, the employer did not ensure that Column F included the necessary description of the injury on the log. 1. On October 11, 2021, an employee was injured while installing a chemical plating tank which resulted in days away from work. The employer did not accurately complete column F of the log indicating the description of the injury that occurred.
