Location - Domiciliary unit (DOM) at 200 Spring Rd., Bedford, MA, 01730 On or about October 1, 2021, the employer did not record five (5) instances, of employee work-related COVID-19 infections on the OSHA 300 log or its equivalent, for the time period between September 4, 2021 and September 27, 2021, resulting in 10 to 19 lost work-days, and one death.
