29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about September 17, 2021, the employer did not report the in-patient hospitalization of an employee who sustained a broken pelvis from being hit by a machine that collapsed within 24 hours. The employee was hospitalized for surgery and OSHA was not notified by the employer at any time.
