29 CFR 1904.29(b)(1): You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at Ariel Corporation in Mount Vernon, Ohio: a. On or about April 20, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. b. On or about May 3, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. c. On or about May 24, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. d.
