29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. W.G. Bill Hefner VA Medical Center, located at 1601 Brenner Ave., Salisbury, NC 28144, on or about September 9, 2021, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2021. a) A specialty clinic employee who tested positive for COVID-19 on August 19, 2021 due to a work-related exposure and had days away from work. b) An emergency department employee who tested positive for COVID-19 on August 16, 2021 due to a work-related exposure and had days away from work. c) A nursing employee who tested positive for COVID-19 on August 11, 2021 due to a work-related exposure and had days away from work. d) A supply chain employee who tested positive on August 2, 2021 due to a work-related exposure and had days away from work. e) An engineering employee who tested positive on July 26, 2021 due to a work-related exposure and had days away from work.
