29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not conduct periodic inspections of energy control procedures at least annually to ensure that the procedure and the requirements of this standard were being followed. Maintenance employees regularly performed servicing and maintenance on equipment that required the use of lockout/tagout. This condition exposed employees to amputation hazards and hazardous energy. Colorado Roofing Products, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1313212, Citation 1 Item 1b, and was affirmed as a final order on July 13, 2018, with respect to a workplace located at 3590 Himalaya Road, Aurora, CO 80011.
