29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about August 20, 2021, near the intersection of 15th Street and Avenue J in Huntsville, TX, one employee was exposed to struck-by and crushed-by hazards when installing a water line in an excavation exceeding a depth of 5 feet without adequate protection from cave-ins.
