29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) Worksite - On or about August 12, 2021, the employer did not notify OSHA within 8 hours of a work-related incident that resulted in a fatality. On 08/16/2021 OSHA was notified of an employee death at this site through a media referral.
