29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a.) Sepco, Inc. - On or about 7/19/21 and at times prior thereto, the company did not report a work related amputation within 24 hours of it occurring. The amputation occurred on or about 7/19/21 at 10:30 AM. The amputation was not reported to OSHA until 7/29/21 at 9 AM. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement
