29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: a. Liberty Oilfield Services, Inc., 9540 E. 104th Ave Henderson, CO 80640: On or before July 21, 2021 the employer did not ensure that employees were protected from falls while working on top of a Hydraulic Horse Power-Pump Truck at heights over 6 feet. On July 21, 2021 an employee sustained injuries after he fell 13 feet and 2 inches to the ground. This condition exposed employees to broken bones and head contusions. Liberty Oilfield Services, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.28(b)(13)(ii), which was contained in OSHA inspection number 1492670., citation number 1, item number 1a and was affirmed as a final order on January 6, 2021, with respect to a workplace located at 9540 E. 104th Ave Henderson, CO 80640.
