29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from collapses by an adequate protective system designed in accordance with 1926 Subpart P. a) Jobsite: 700 Ramona Ave, Philadelphia, PA - On or about July 26, 2021 the employer did not ensure employees working in front of an approximately 20 foot tall shear excavated wall were protected from collapses by the use of an adequate protective system. The employer allowed the use of 8' x 14' tall steel plate against the excavated wall, which was inadequate for protection against collapses during retaining wall construction activities, exposing employees to collapse hazards. b) Jobsite: 700 Ramona Ave, Philadelphia, PA - On or about July 27, 2021 the employer did not ensure employees working in front of an approximately 20 foot tall shear excavated wall were protected from collapses with the use of a protective system. An employee was working within 3 feet of the unprotected wall as it collapsed, thus killing the employee.
