29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. (a) Facility - On or about July 15, 2021, a work related fatality was not reported to OSHA within 8 hours. The fatality was reported on July 21, 2021.
