29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about June 6, 2021, the employer did not contact OSHA within 24 hours of when an employee sustained a work-related injury, suffered an electric shock injury with exposed and live wires, that caused an employee an in-patient hospitalization resulting loss of two fingers to the right hand.
