29 CFR 1926.501(b)(4)(i): Each employee on a walking/working surface was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite, 4201 Grand Avenue, Neville Island, PA: On or about June 17, 2021, during roofing work, the employer did not ensure that appropriate fall protection measures were taken while working near skylights, exposing workers to a fall hazard of approximately 30 feet.
