29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about April 28, 2021, an employee was exposed to amputation hazards due to lack of adequate point of operation machine guarding on Spot Welder #30. Lyon, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1482899, citation number 1, item number 2 and was affirmed as a final order on November 3, 2020 , with respect to a workplace located at 475 N. Veterans Parkway, Watseka, IL 60970.
