Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) The Hyatt House at 17254 Five Points Square Lewes, DE 19958: On or about May 13, 2021, the employer failed to ensure employees had and used fall protection while working approximately 41 feet above the grade, exposing employees to fall hazards.
