29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: (a) On or about April 6, 2021, located at the west end of a job site located West of Sentry Drive on West Sunset Drive, Waukesha, WI, a one (1) inch diameter cable located approximately five feet below the ground level and next to the West end of the yellow National Trench Safety trench shield box, was not protected, supported or removed to protect employees from potential exposure to the cable.
