29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident. COMPANY-WIDE: On or about March 23, 2021, the employer was advised that an employee had died of COVID-19 the previous day. The employee had been caring for a client who tested positive for COVID-19 on March 1, 2021. The employer did not report the work-related fatality to OSHA within 8 hours.
