29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Fall River Jewish Home Inc. located at 538 Robeson Street, Fall River, MA: a. On or about January 8, 2021, an employee was hospitalized as a result of a confirmed case of COVID-19 and died on January 27, 2021. The employer did not record the work-related fatality on the OSHA 300 log or its equivalent for the calendar year 2021. b. On or about December 27, 2020 to December 31, 2020, the employer did not record the COVID-19 workplace illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: There were eight incidents of COVID-19 infection that resulted in 10 to 14 days away from work; c. On or about January 1, 2021 to January 26, 2021, the employer did not record the COVID-19 workplace illnesses on the OSHA Form 300 or equivalent for the calendar year 2021: There were seventeen incidents of COVID-19 infection that resulted in 10 to 14 days away from work.
